REGISTRATION FORM
To be a fully registered patient you will have to attend an initial checkup visit and only after the dentist will do a full dental assessment will you be registered as an NHS or private patient.
To start the registration, we need you to fill in the form given below, after which we will give you a call to schedule your first appointment. In addition, we’ll require your date of birth, address (including postcode), and general doctor practice name in the message.

Thursday – 9:00 am to 5:00 pm
